Effect of Long-term Treatment and Effi cacy of Transdermal Opioid Patch in the Chronic Low Back Pain

Abstract
Background: It is known that chronic low back pain may hardly be cured with pure analgesic, and recently, it is suggested that strong opioid analgesics should be used also in early treatment. This study purposes to fi nd out the effi cacy of transdermal opioid patch to the chronic low back pain patients and the possibility of addiction and abusing after long-term treatment more than six months.

Methods: The Subjects are 179 cases of patients with chronic low back pain that claimed pain over visual analogue scale (VAS) 7 in spite of pure analgesic from March 2008 to June 2009; in particular, 14 cases using transdermal opioid patch more than six months are analyzed whether there was addiction and abusing. The average of the subject is 61.9 years old (33-90), and the major diagnosis was spinal stenosis (39 cases, 57.4%). VAS before and after applying the patch, the existence of complication, the drug use questionnaire developed by Compton et all in order to check for addiction and abusing were analyzed.

Results: The group using transdermal opioid patch more than three months was 33 cases and the average VAS decreased from average 9.0 before the administration and average 3.3 after the administration. In 18 cases using the patch less than three months, the VAS decreased from the average 8.9 before the administration to 3.5 after the administration. The difference in VAS according to the existence of previous operation, sex and age was not statistically meaningful. The group using the patch over six months was 14 cases in total; 5 cases were male and 9 cases were female. The average age was 65.5 years old (53-80), and the average VAS decreased from 9.4 before the administration to 2.6 after the administration. There was no case that showed the tendency of addiction and abusing.

Conclusions: In the treatment for patients with chronic low back pain, the long-term use of transdermal opioid patch was useful for pain control and there was no grave side effect.
